APOSTILLE ON BIRTH OR DEATH CERTIFICATE - COVID-19 UPDATE

Before Covid-19, we were providing Apostille services for our clients. To do this, we used the NYS Department of State walk-in Apostille Services downtown at 123 Williams Street. Since the shutdown, this walk-in service has been discontinued and we can find no plans to reopen this walk-in office in any of the reopening phases.

Apostille must now be obtained from Albany, NY by mail.

Birth Certificate- Death Certificates
You must first get a long-form birth or death certificate.

If your Birth, Marriage and Death certificate was issued by New York City Department of Health, you must take it and your letter of exemplification to the Country Clerk's office for certification. This cost $3 and all types of payment are accepted. The Court Clerk's office is opened 9AM - 5PM.

• You must include a return stamped envelop addressed to yourself and from yourself.
  For faster service use Priority Mail to send your Apostille request and for your return self-addressed stamped envelop.

Send the form along with the certificate and the letter of exemplification and method of payment (instructions contained in the above form) and your self-addressed stamped envelop to the following address.
